The online form where you will upload all requirements specific to each Award you are applying for. This also includes your basic identifying information and your Letters of Attestation.
Official High School Transcript
A high school transcript is a record of all your academic accomplishments in high school.Official versions are often sent directly by your high school, either through snail mail or as an e-mail. Ask your school counselor or another school official to release your transcript.
Involvement with at least 2 Sponsoring Organizations
A total of 30 volunteer hours are required for many of the awards. You can acquire the total hours throughout your high school career, as early as your freshman year. See this page for a list of participating organizations.
When you volunteer at events, you must bring with you a printed copy of the Letter of Attestation to have signed by the individual overseeing the event.

Interview with JACGP
Some awards require an interview with JACGP committee members.
Once you’ve applied for such an award(s), you will be contacted by the JACGP and then coordinate a time to hold the interview at a designated location.
Expressive Work or Artwork
Some awards require an an expressive work or artwork as part of the application. Please see the specific awards’ requirements for more details on the stipulations of the works you may submit.
Essay
Some awards require an an essay as part of the application. Please see the specific awards’ requirements for more the prompt specific to that award.
Past/Present Judo enrollment
One award offered (the DBT Judo Dojo Sensei Memorial Scholarship) requires present or past enrollment in a judo program.
